What is Speech & Language Delay?

Speech and language delay refers to a child whose communication development is significantly behind what is typically expected for their age. Speech delay involves difficulty producing speech sounds clearly; language delay involves difficulty understanding or using words, sentences and concepts. Both are distinct but often co-occur.

Speech and language delay is one of the most common developmental concerns in young children. Causes can include hearing loss, oral-motor difficulties, cognitive delays, autism spectrum disorder, environmental factors, or simply a natural variation in development. Early assessment and intervention are critical — the first five years are the most important window for language learning.

Signs to Look For

Different signs present at different stages of development. Here's what to watch for.

Ages 0–2

Infant Signs

  • Not babbling by 9–12 months
  • No first words by 15 months
  • Not using 2-word combinations by 24 months
  • Limited response to their name
  • Not pointing to objects or waving by 12 months
  • Doesn't follow simple instructions
  • Limited eye contact during interaction
Ages 3–6

Preschool Signs

  • Speech is very difficult to understand
  • Uses fewer words than peers
  • Relies on gestures rather than words
  • Difficulty following multi-step directions
  • Struggles to tell simple stories or sequences
